Using D-ID\u2019s <a href=\"https:\/\/www.d-id.com\/personal-avatars\/\">generative AI avatar<\/a> technology, SIU developed \u201cRandy,\u201d a virtual patient designed to help medical learners practice, make mistakes, build confidence, and, most importantly, learn to earn trust.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Today, Randy\u2014and a growing roster of virtual patients\u2014are being used to safely simulate medical scenarios that students would otherwise only experience for the first time during high-stress clinical rotations. <\/span><span style=\"font-weight: 400;\">After trying various virtual training options, SIU turned to a smarter, more human solution: an AI-driven program that reshapes how students develop clinical skills and bedside manner.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">The Challenge<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">SIU Medicine originally set out to create a gamified training experience for its medical students, but quickly realized that a game engine couldn\u2019t accommodate the unpredictable nature of real medical learners.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cWe realized that you can\u2019t orchestrate a video game for medical students,\u201d said Dr. Richard Selinfreund, associate professor and head of the pathology group at SIU. \u201cThey ask the right\u2014but unexpected\u2014questions, and the whole thing crashes.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">On top of that, Dr. Selinfreund was tasked with something games couldn\u2019t replicate: helping future doctors learn to build trust with their patients\u2014arguably one of the most vital (and most overlooked) clinical skills.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The team needed something smarter, more dynamic, and deeply human. They realized the future of medical education wouldn\u2019t be powered by games\u2014it would be powered by AI.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">The Solution<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">SIU Medicine turned to D-ID to bring its <\/span><a href=\"https:\/\/www.wpsdlocal6.com\/news\/meet-randy-rhodes-the-ai-patient-changing-medical-training-at-siu\/article_6308b7a2-e8a1-11ef-8efb-afd5c846df22.html\"><span style=\"font-weight: 400;\">virtual patients to life<\/span><\/a><span style=\"font-weight: 400;\">. With D-ID\u2019s Real-Time embedded agents, the team built out a cast of hyper-realistic AI patients powered by a custom large language model (LLM) backend. These avatars simulate conversations with medical learners, adapting in real time to student input and presenting a wide variety of symptoms, conditions, and emotional states.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The flagship patient, \u201cRandy Rhodes,\u201d was designed with different difficulty levels\u2014from beginner to advanced\u2014and trained using documents uploaded into SIU\u2019s custom RAG (retrieval-augmented generation) system. Voices were customized using ElevenLabs to reflect each patient\u2019s condition\u2014sick, short of breath, or fatigued\u2014creating a more immersive and empathetic experience.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The result: an intelligent, responsive AI patient that students can access on their phones, at their own pace, and as many times as they need to build confidence before ever entering a real exam room.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">Real Impact: Reducing Anxiety and Accelerating Competency<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">One of the most challenging skills for new medical students is learning how to take a patient history. It\u2019s also one of the most crucial. \u201cThere\u2019s no such thing as a bad history,\u201d Dr. Selinfreund explains, \u201cjust bad history-takers.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">By giving students a chance to practice in a safe environment, SIU is helping them build trust, reduce anxiety, and develop critical clinical instincts before the stakes are real.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Students have responded enthusiastically. Some even asked to replace parts of the traditional curriculum with the AI simulations. \u201cA 24-year-old told me, \u2018I get to see every disease and practice before I get to the emergency room,\u2019\u201d says Selinfreund. \u201cThis generation grew up on phones and screens\u2014this is how they learn.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Emergency departments have also taken notice, with residency directors seeing the potential for what SIU calls an \u201cacceleration to competency.\u201d Rather than starting from scratch, incoming residents arrive having already practiced core diagnostic conversations.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">Scalable, Trusted, and Ready for Growth<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">SIU\u2019s in-house prompt engineering team built the patient personas and backend integrations but relied on D-ID for avatar creation, infrastructure, and support. \u201cWe\u2019re constantly evaluating other platforms,\u201d said Selinfreund, \u201cbut one thing D-ID is crushing it on is service. I wouldn\u2019t take any other company into this 10-university alpha trial.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The large-scale pilot, launched on March 10, brings Randy to medical students worldwide and is expected to generate millions of interactions. To prepare, Dr. Selinfreund routinely stress-tests the system, running Randy through varied scenarios to ensure stability at scale. \u201cWe\u2019re about to publish this work, and I need a robust infrastructure and a team I can trust,\u201d he said. \u201cThis is a global study\u2014I can\u2019t afford for Randy to crash.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Separately, the team collects anonymized feedback through REDCap and publishes academic studies to validate outcomes. The next generation of avatars may include body language, hand gestures, and more expressive behaviors to further boost realism and trust-building.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The ultimate goal? Train doctors not just to diagnose, but to connect. \u201cWe measure whether medical learners are developing trust with the patient,\u201d says Selinfreund. \u201cThat\u2019s the goal.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">As the university moves toward spinning out a company to support and scale this effort, SIU continues to push the boundaries of what\u2019s possible in medical training, with D-ID by their side.<\/span><\/p>\n\n\t<\/div>\n\t\t\t\t\t\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t\t\t\t<\/div>\n\n\t\t\t\t\t<\/div>\n\t<\/div>\n<\/section>\n\n\n\n<figure class=\"wp-block-pullquote\" style=\"border-top-width:1px;border-bottom-width:1px;font-size:16px\"><blockquote><p>&#8220;There\u2019s no way I can train a medical learner with a chatbot.
